Ethnic Variations in Patterns of Left Ventricular Hypertrophy on Cardiac Magnetic Resonance Imaging in a Multi-Ethnic New Zealand Population with Hypertrophic Cardiomyopathy

Background: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiac disorder. We describe the clinical and imaging characteristics of patients at Te Whatu Ora Counties Manukau with HCM who underwent cardiac magnetic resonance (CMR) imaging.

160 patients (102 men, 58 women) with HCM who underwent CMR between August 2017 and September 2024 were identified from the Aotearoa New Zealand All Cardiology Services Quality Improvement (ANZACS-QI) CMR registry. The distribution of left ventricular hypertrophy (LVH) was classified into 4 patterns: (1) isolated septal hypertrophy; (2) isolated apical hypertrophy; (3) mixed apical/septal hypertrophy; (4) other hypertrophy patterns.

Pacific patients were younger (44.8 ± 12 years, p = 0.003) compared to others. Isolated septal hypertrophy was the most common pattern in all groups: Māori (64.3%), European (61.9%), Pacific (47.2%), Asian (40.7%). However, there was variation in other patterns of LV hypertrophy by ethnicity (p=0.0078). Mixed apical/septal hypertrophy was more common in Asian (29.6%) and Māori (25.0%) than Pacific (11.1%) or European (9.5%). Other LVH patterns were more frequent in Pacific (38.9%) compared to others.







Conclusion: The pattern of LVH varied by ethnicity in our patients with HCM who underwent CMR. Reasons for variation may include genetic, epigenetic and environmental factors. There are implications for screening, particularly in Māori, Pacific, and Asian peoples where mixed and other patterns of LVH appear more commonly and at a younger age in Pacific peoples. Our data underscores the importance of patient access to CMR to enable accurate diagnosis and therefore earlier treatment of HCM.
