Novel Multiplayer Collaborative Extended Reality Technology for Global Knowledge Sharing in Pediatric Cardiac Care - International Feasibility Study

Background: Extended Reality (XR) technologies have transformative potential in healthcare applications.

We explored feasibility of using XR for collaborative visualisation and analysis of 3D digital heart models by remotely located pediatric cardiac professionals for knowledge-sharing, morphological understanding, treatment planning and remote training .

Methods: Seven pediatric cardiac teams from developing world (one each from Brazil, Malaysia, Vietnam & four centers across India participated. Each site was provided two MetaQuest3 VR devices with collaborative XR software (MedisphereXR, India), creating a Mixed Reality ‘3D-model viewing’ system capable of multi-player remote collaboration (using Wifi internet). 3D digital twins of selected complex CHD modelled from tomographic data were imported into system for collective viewing.

Participants simultaneously logged into virtual rooms using personalized passcodes and were able to see, hear and actively interact with each other as lifesize digital avatars while simultaneously viewing and interacting with 3D heart-models with custom digital tools

(dissection, measurement, annotation etc). Case discussions were led by an expert. Post-

session feedback was obtained.

Results: Feasibility studies were successfully completed with six collaborative XR meetings (between December 2024-June 2025) involving all teams. The novel technology allowed seamless immersive collaboration, collective viewing of cases and active engagement despite large geographical distances. Mixed Reality format with avatars and virtual hearts being projected into real-world environment evoked a unique sense of togetherness. Ability to converse (inbuilt microphones and speakers) and physically move and interact through physical spaces made the interaction immersive and realistic.

Feedback showed: 1). 100% participants felt that XR enhanced their learning experience and understanding of the complex cases, and their ability to plan treatment. 2). Allparticipants perceived ‘presence’ and ‘engagement’ with other participants in the virtual space.

Conclusion: XR technologies can be meaningfully used for borderless collaboration and knowledge sharing between pediatric cardiac professionals across the globe.
